Why do capable boards sometimes make poor decisions?

I recently spoke to Dr Sabine Dembkowski on the Better Boards podcast about the issues raised in my upcoming book, ‘Why Companies Fail: Exposing Human Performance in the Boardroom‘.
We discussed what happens to people and teams in the boardroom, and why factors such as leadership, communication, challenge and group dynamics can have a significant impact on the quality of decisions. Is blaming people more important than finding out why it happens?
Thanks to Sabine for a very interesting conversation.
